Health
Insurance: PPO,
base plan 100% paid by employer. Additional family members
can be added at employees expense.
Basic
Life: - $15,000.00, premium paid by employer.
Dental
Insurance: Premium by employer. Additional
family members can be added at employee's expense.
401K
Plan: Principal Group.
Employer contribution.
Long
Term Disability: 60% of income covered,
premium paid by employer.
3 Month
waiting period for health, dental and 401K participation
6 Month
waiting period for long term disability.
Payroll:
Semi-monthly. Paydays are on the 15th day of
the month, if falls on a weekend, paid on Friday before and
the last working day of the month. Direct deposit available.
